I normally start with an idea, and then I think, "Oh this would look good here," or "I wonder what this looks like." Often my OCs often turn out much different than what I thought they'd look like when I was done.

Good Topic!
I usually plan it out a few weeks in advance all in my head and then, when I have time, start building. I often build lots of the small details before I've got the actual building built, and work them in as I go.
I rarely spend much time looking for peices, because mine are all sorted colorwise and sometimes type-wise, in the case of castle parts (prefab walls, BURPs, plates, two by ones).
After the MOC is built, I, like Josh, sit it on a shelf for a while, and usually then tear it down and improve what I can by building from the ground up again.

For me, its a question of size.
Smaller MOCs like this, or this are scratch-built. Larger MOCs likethis are a different story. There is simply too much that can go wrong to build a huge MOC without some planning. You simply leave too much to chance when building a large MOC without a battle plan.
Heck, even with blueprints, brick counts and other strategies, things can still go wrong. My current castle has been in development for the better part of six months, and it probably won't be finished anytime soon! If I hadn't done any planning, I probably would have given up all together.
Later.
