What custom pieces were you thinking of? Things like figs and accessories are addressed in the rules.

We have always allowed people to used a mosaic-type image to show action features (e.g. a secret door) - I'd say this would be your best bet. I'm concerned that others might abuse the idea of videos to essentially skirt the three photos rule.

If either the Peeron or Bricklink inventory lists extra parts, those are okay. Let's face it, these are usually like a couple of 1x1 plates, so probably don't make a huge difference.
On your second question, let your imagination run wild. If you think back to the old set boxes, they would have pictures alternate models on the back using just the pieces in that box.
