Nonono, that's the wrong thing. I'm talking about the ones that accompany the long capes. To put the custom shoulder extensions onto a minifig, you just put the center hole onto the neck first, then put both side holes on over that.

Those are the only designs there, actually.
Although there is a shoulder cape for the sword one, and full sized versions, the size of the template of the cape.
