Basically, it's like a really small version of the normal, two-holed capes, such as the Jedi and Crownie King wears: the two holes are folded over each other. In this particular case, the shoulder cape would then be worn to the side, over a shoulder (presumably in conjunction with another on the other shoulder... but maybe I shouldn't presume).
Nonono, that's the wrong thing. I'm talking about the ones that accompany the long capes. The double pauldrons or whatever they're called...

To put the custom shoulder extensions onto a minifig, you just put the center hole onto the neck first, then put both side holes on over that.
Feel free to use the template designs so long as credit is given:
-Long Cape-
-Shoulder Extension-
If anybody ever wants one of their custom designs printed in fabric, just let me know.

Here are a few of my design ideas, although they will be made on paper before deciding what to use on fabric.

Those two are great! Love the blade one especially (I had been trying to come up with a decent blade style a while back, never finished it, though).
Folder is un-public now, so I guess I'll wait to see the rest.
Keep up the fabulous work!

Those are the only designs there, actually.
Although there is a shoulder cape for the sword one, and full sized versions, the size of the template of the cape.
